Case study: Fred, Integrated Frailty Service patient

Our integrated frailty system provides holistic, individualised care to residents requiring support at a time when they need it most.

The service is made up of a number of teams and settings who work closely together so that we can identify an individual living with frailty as early as possible, and then implement a support plan tailored to that person, with on-going support as required.

We consider a person’s time the most important currency and exploring what matters most to an individual is at the forefront of our working ethos. We are here to support people when in need and to promote independence.
